This page explains what Dental Art on Harbord collects through this website, why, and who else handles it. It covers the website. Your dental records are looked after separately, as Ontario's health privacy law and the Royal College of Dental Surgeons of Ontario require.
What you send us
When you use the appointment request form, we receive what you type in: your name, phone number and email if you give one, whether you are a new patient, what the visit is about, how you are covered, the days and times that suit you, and any message you add. We also see which page you sent it from.
Please keep the form brief and leave out detailed medical information, because it reaches us by email. If you are in pain, call us at 416-530-1447 instead.
How we use it
We use your request to call you back and arrange your visit, and to answer your question. If you give an email address, we send one confirmation that your request arrived. We do not sell your information, and we do not add you to a marketing list because you sent a request.
Who else handles it
- Resend delivers the form to our office and sends your confirmation email.
- Google Workspace hosts our office email, where your request arrives.
- Vercel hosts this website and, like any web host, processes technical details such as your IP address.
- Google Analytics and Google Ads tell us how people find and use the site, and whether our advertising works. They use cookies and record things like pages visited, and whether someone tapped to call or sent a request. They do not receive what you type into the form.
- Google Maps, Google Fonts and YouTube supply the map, the lettering and the videos on some pages, so your browser connects to Google when it loads them.
- WhatsApp applies its own privacy terms if you choose to message us there.
Some of these companies store information outside Canada, including in the United States, where it is subject to local law.
Cookies and your choices
You can block or clear cookies in your browser settings, and the site will still work. Google also offers a browser add-on to opt out of Google Analytics and settings for the ads you see.
How long we keep it
If your request leads to treatment, what we need becomes part of your dental record, which we keep for as long as the rules for Ontario dentists require. Other requests are kept only as long as we need them to respond and for ordinary office records.
Your rights
You can ask to see the personal information we hold about you, ask us to correct it, or withdraw your consent to be contacted. To do any of these, or to ask a question, contact Dr. Angela at appts@dentalartonharbord.com, 416-530-1447, or 382 Harbord St, Toronto, ON M6G 1H9.
If we can't resolve a concern, you can complain to the Information and Privacy Commissioner of Ontario or the Office of the Privacy Commissioner of Canada.
